Onboarding of Clients & Start of Service.
- Once you have received an authorization for service from the MLTC or MCO, we will complete the following activities to ensure the smooth start and continuation of care:
- Nursing Assessment - Our nurses meet with every client and their caregiver to conduct a home care assessment and for an in-depth discussion of their care needs. This allows us to carefully tailor our services to each client's specific care concerns and challenges. We determine when home care is needed and what level of care you or your loved one needs. During a home care assessment, our nurse will meet with you to discuss your greatest age-related or disability-related challenges, your lifestyle and daily schedule, and your overall goals for care. This comprehensive assessment includes gathering information concerning the patient's individual physiological, psychological, sociological, and spiritual needs. It is the first step in the successful evaluation of a client.
- Caregiver selection process - Getting to know you is a key part of the caregiver selection process. For JASACare, a full in-home care assessment means that care coordinators can match you with a caregiver whose personality, skills, experience, language, and cultural awareness are best suited to the client. Thanks to our in-depth care assessments, we can suggest matches that closely meet our clients' needs, making it easy for our clients to thrive in their care.
